Healthcare Foundation Announces $365,000 in New Funding to Strengthen Northern Sonoma County’s System of Care Through Regional Coalition

Healdsburg, CA, March 12, 2026 – The Healthcare Foundation Northern Sonoma County today announced $365,000 in new grant funding from Sutter Health ($250,000) and the North Sonoma County Healthcare District ($115,000) to support the Northern Sonoma County Coalition, a collaborative initiative designed to strengthen the region’s safety-net providers as they transition into California’s evolving Medi-Cal reimbursement system under CalAIM.

Launched in response to California Department of Health Care Services’s California Advancing and Innovating Medi-Cal (CalAIM) initiative, the Coalition brings together six trusted local nonprofits and clinics—Alexander Valley Healthcare, Alliance Medical Center, The Botanical Bus, Corazón Healdsburg, La Familia Sana, and Reach for Home—to build shared infrastructure, align service delivery, and increase access to reimbursement for services that address the social drivers of health, including housing navigation, food access, transportation, and case management. This new funding comes on the heels of a strong and productive first year in which all members voiced the benefit of the Coalition for their organizations and the communities they serve.

While CalAIM represents a historic opportunity to integrate community-based services into reimbursable healthcare delivery, the transition process is complex, particularly for smaller, grassroots providers. Through bimonthly convenings, technical assistance referrals, and coordinated systems support, the Coalition ensures that local organizations can successfully become or maintain CalAIM provider status, unlocking sustainable funding streams for services that are essential to community health.

These funds are specifically earmarked for the CalAIM initiative which was launched in 2025 under a separate grant funded by the District. The Northern Sonoma County Coalition, convened by the Healthcare Foundation since January 2025, reflects a shared regional commitment to collaboration, mutual aid, and long-term health equity—ensuring that local providers are equipped to meet community needs today and into the future.
