Community Response Teams

In 2022, the Healthcare Foundation distributed funds secured to support the development of local Community Response Teams (CRTs). Funding supported the development of highly localized plans by sub-region or “zone” to increase vaccine outreach and health equity across Sonoma County, with a focus on people from low-income, marginalized populations, and people of color. The long-term goal is to build a public health infrastructure in partnership with the community to support response, recovery, and resiliency when faced with public health threats.

Participants

Building on the successful model in neighboring Marin County, the Healthcare Foundation identified five community-based organizations as lead agencies to develop CRT plans in Sonoma County:

Corazón Healdsburg
La Luz
Petaluma Health Care District
Center for Well-Being
River to Coast Children’s Services
